Identifying the real problem


We have all worked in organisations where clear problems continue to occur because managers fail to get to the root of the issue and continuously apply quick fixes.  

This is when root cause analysis such as the 5 why’s can help managers and leaders continue to question issues they face. They can then solve the root cause and the issue should not come back.  

The first step is to assemble the team, this includes everyone from shop floor to board level.  

Next you need to define the problem you are being faced with.  

Once this is done you need to ask yourself why the issue is happening so you can collect the information.  

From this you need to pick one of the statements or causes and ask yourself why 5 times.
